By signing below,
- I give LUCKY HORSESHOE TATTOO the right to use my tattoo and/or piercing in any publication or advertisement to help in the promotion and recognition of the studio.
- I certify that I am 18 years old or older and have provided proper ID.
- I have received a copy of applicable written care instructions and I have read and understand such written instructions.
- I hereby release Lucky Horseshoe Tattoo and its associates from any and all liability and negligence due to aftercare.

All artists work on a commission basis and are not considered employees, nor employed by Lucky Horseshoe Tattoo. Each Tattoo/Piercing artist is fully responsible for any & all work they do. Lucky Horseshoe Tattoo reserves the right to refuse to tattoo or pierce anyone who appears to be under the influence of alcohol and/or drugs.
